Benefit Paid By Eligibility You Receive
401(k) Plan

SOURCE & You

1st of the month following completion of ninety (90) days of credited service.

SOURCE will determine at the beginning of each plan year what the discretionary match will be. You may contribute up to 100% of your compensation on a pre-tax basis to the 401k Plan, up to Federal limits. Fifty percent (50%) vesting after one year. One hundred percent (100%) vesting after two years.

Medical Plan

Upon Hire

United Healthcare. Open access plan, no primary care physician to choose and no referrals are necessary. Includes prescription plan with a mail order option. SOURCE pays a significant portion of your premiums. Your portion of premium is paid on a pre-tax basis.

Dental Plan

SOURCE & You

Upon Hire

Choice of an Indemnity or DHMO Plan. Coverage includes routine, preventive, and restorative treatment. There is $1,000 annual maximum benefit for the Indemnity Plan. With DHMO there is no annual maximum.

Life Insurance

One (1) times your base salary.

Additional Life Insurance

Up to five (5) times your base salary, up to a maximum of $500,000.

Accidental Death & Dismemberment

Short-Term Disability

60% salary replacement after 5 days of disability.

Long-Term Disability

66 2/3% salary replacement after 26 weeks of disability, $10,000.00 maximum monthly benefit.

Health Tax-Saver Select Account

Upon Hire

You may reduce your salary up to $5,000 per year to be used for reimbursement of approved health care expenses, not covered by the medical plan.
